Stroke is indeed a significant cause of both mortality and morbidity globally. Large artery atherosclerosis is responsible for around 20% of all ischemic strokes among the different types of strokes. Approximately 50% of cases within this category are intracranial, meaning the blockage or narrowing of arteries occurs within the brain. Unfortunately, even with medical treatment, intracranial large artery atherosclerosis carries a relatively high recurrence rate. This highlights the need for effective interventions and management strategies to address this condition and reduce its impact on individuals' health.
In addition to large artery atherosclerosis, intracranial stenosis is a notable contributor to ischemic strokes. It accounts for approximately 10% of ischemic strokes in the U.S. and can be even more prevalent, reaching up to 50 percent in certain regions of Asia. The rising incidence of Intracranial Atherosclerotic Disease (ICAD) may be higher than reported, as current diagnostic methods may not detect symptomatic, non-stenotic atherosclerotic plaques. Autopsy analyses of stroke patients have revealed many such plaques. Interestingly, around 6% of these plaques were found to have superimposed thrombi, indicating a potential association with ischemic events.
Studies show differences in the prevalence of ICAD among various racial and ethnic groups. Hispanics, Africans, and Asians are more likely to have higher ICAD rates than other ethnic backgrounds. These differences may be due to various factors, including varying risk factor profiles, dietary habits, and genetics. For instance, African American patients have a higher prevalence of hypertension, hyperlipidemia, and diabetes mellitus, which are linked to this group's higher prevalence of ICAD. These risk factors contribute to the development and progression of atherosclerosis, leading to intracranial atherosclerotic plaques.
A research study published in the Radiological Society of North America examined 169 patients and found that 20 individuals (11.8%) experienced complications. Among these individuals, 10.5% suffered from strokes. Four patients had symptomatic intracranial hemorrhages (ICHs), and six had ischemic stroke. Unfortunately, two of these patients died due to complications. Additionally, two patients had asymptomatic ICHs, one experienced a transient ischemic attack, and another had an asymptomatic dissection. These findings demonstrate the occurrence of various complications associated with intracranial stenting procedures, including both symptomatic and asymptomatic events such as strokes and hemorrhages.
