The collaborative care model is a type of integrated care designed to treat common mental health conditions such as depression and anxiety. It is also useful for people who have suffered a lot and have disturbed mental health due to alcohol or substance abuse. The collaborative care system offers primary care for behavioral health with several integrated care systems to improve patients' mental health. Collaborative care model uses a team-based, interdisciplinary approach to deliver evidence-based diagnoses, treatment, and follow-up care to an identified patient population. By deployment, the collaborative care model works effectively using three applications such as web-based, cloud-based, and on premise. A key difference between cloud and web applications is architecture. A web application or web-based application must have a continuous internet connection to function. Conversely, a cloud application or cloud-based application performs processing tasks on a local computer or workstation. An internet connection is required primarily for downloading or uploading data. Whereas, in an on-premises environment, resources are deployed in-house and within an enterprise’s information technology (IT) infrastructure. An enterprise is responsible for maintaining the solution and all its related processes.
